Key Responsibilities
• Manage a caseload of clients, providing consistent remote support and case management.

• Monitor client progress and adjust care plans as needed, ensuring effective outcomes.

Requirements
• Bachelor's degree in Social Work, Psychology, or a related field.
• Minimum of 4 years of experience in case management or social services.
• Proven ability to conduct assessments and develop effective care plans.
• Excellent proficiency with remote work tools, video conferencing, and digital case management software.
• Strong organizational skills and the ability to manage multiple priorities effectively.
