Remote Case Manager - Addiction Services

About the Role

Our client is seeking a dedicated Remote Case Manager to join their expanding Addiction Services team, supporting individuals across Durban and beyond. This position is fully remote, allowing you to provide crucial support and guidance to clients seeking recovery from substance use disorders from the comfort of your home office. You will play a key role in developing individualized treatment plans, coordinating care with healthcare providers, and empowering clients on their journey towards sustained sobriety and improved well-being. Key Responsibilities

• Conduct comprehensive assessments of clients' needs related to addiction and co-occurring mental health issues.
• Develop, implement, and monitor personalized treatment and recovery plans .
• Coordinate client care with physicians, therapists, and other healthcare professionals to ensure integrated support.
• Provide ongoing counseling, crisis intervention, and support to clients and their families via virtual platforms.
• Assist clients in accessing resources such as housing, employment, and support groups.
• Maintain accurate and confidential client records in accordance with ethical and legal standards.

Requirements

• Bachelor's Degree in Social Work , Psychology, Counseling, or a related field.
• Professional registration with relevant bodies (e.g., SACSSP, HPCSA).
• Minimum of 2 years of experience in addiction counseling or case management.
• Strong understanding of addiction, recovery principles, and mental health support.
• Proficiency in virtual communication tools and electronic health record systems.
• Excellent empathy, active listening, and advocacy skills .
