About the Role

This position is based remotely, allowing you to contribute from anywhere within South Africa while focusing on clients in the Centurion, Gauteng area and surrounding regions. You will play a critical role in coordinating care, providing vital support, and ensuring access to health services for individuals and families. This is a remote-first role, emphasizing the use of technology to connect, assess needs, and manage cases effectively. You will be responsible for developing personalized care plans, connecting clients with appropriate healthcare providers and community resources, and monitoring their progress towards health goals.

Key Responsibilities

• Conduct comprehensive remote client assessments via telehealth platforms.
• Develop individualized care plans tailored to specific health needs and goals.
• Coordinate medical appointments, referrals, and follow-up care.
• Connect clients with appropriate community health resources, social services, and support groups.
• Provide ongoing support, education, and advocacy for clients and their families.
• Monitor client progress, document interventions, and evaluate outcomes.
• Liaise effectively with healthcare providers, social workers, and other stakeholders.
• Maintain accurate and confidential electronic health records.
• Adhere to all relevant ethical and professional standards and regulations.
• Participate in team meetings and case conferences, contributing to collaborative problem-solving.

Requirements

• Registered Nurse (RN), Social Worker (MSW), or equivalent degree in a health-related field.
• Minimum of 3-5 years of experience in case management, community health, or clinical practice.
• Proven ability to manage a caseload effectively in a remote setting.
• Excellent telehealth communication and interpersonal skills.
• Strong understanding of the healthcare system and community resources.
• Proficiency with electronic health record (EHR) systems and virtual collaboration tools.
• Demonstrated ability to work autonomously and manage time efficiently.
• Empathy, resilience, and a strong commitment to client advocacy.
• Access to a reliable internet connection and a suitable home office environment.
• Professional registration with the relevant South African healthcare council.
