Our client is seeking a highly organized and dedicated Remote Community Case Manager to join their fully remote team, serving the Centurion area. In this critical role, you will be responsible for coordinating and managing a caseload of clients, ensuring they receive the necessary support services to achieve their personal goals and enhance their well‑being. You will leverage technology and strong communication skills to connect with clients, assess their needs, and develop comprehensive care plans. Key Responsibilities
- Conduct remote client assessments via phone and video conferencing to identify needs and strengths.
- Develop, implement, and monitor individualized case management plans for a diverse client population.
- Coordinate access to community resources, including healthcare, housing, employment, and social services.
- Maintain accurate and detailed client records in compliance with agency policies and data protection regulations.
- Regularly communicate with clients, their families, and service providers to ensure continuity of care.
- Advocate on behalf of clients to secure appropriate services and support.
Requirements
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
- Minimum of 3 years of experience in case management or a similar client support role.
- Proficiency with case management software and virtual communication tools.
- Strong understanding of social service systems and community resources.
- Excellent organizational, time management, and problem‑solving skills.
- Ability to work autonomously and maintain professional boundaries in a remote environment.
