About the Role

Key Responsibilities

• Conduct virtual needs assessments to understand clients' health service requirements and barriers.
• Provide telephonic and online guidance on navigating healthcare systems, insurance, and available programs.
• Assist clients in scheduling appointments with healthcare providers and accessing necessary medical services.
• Educate community members on preventive health practices and available wellness resources through digital channels.
• Maintain accurate electronic health records and case management documentation.
• Collaborate remotely with healthcare providers, community organizations, and internal teams to ensure comprehensive client support.

Requirements

• Bachelor's degree in Public Health, Social Work, Nursing, or a related field.
• Minimum of three years of experience in community health, patient advocacy, or health navigation.
• Proficiency with telehealth platforms, virtual communication tools, and electronic health record systems.
• Strong understanding of the South African healthcare landscape and public health initiatives.
• Excellent verbal and written communication skills, with the ability to convey complex information clearly.
• Must possess a reliable internet connection and a dedicated home office setup conducive to remote work.
