About the Role

Our client is looking for a dedicated Remote Social Support Facilitator to join their expanding Community & Social Care department. You will play a key role in helping individuals and families access resources, manage challenges, and improve their overall well‑being. Key Responsibilities

• Provide remote one-on-one support to clients via video conferencing, phone calls, and secure messaging platforms.
• Assist clients in identifying their needs and developing actionable plans to achieve their goals.
• Guide clients in accessing community resources, social services, and support groups.
• Offer empathetic listening and emotional support to clients facing various life challenges.
• Educate clients on available resources and empower them to utilize them effectively.
• Maintain accurate and confidential electronic records of client interactions and progress.

Requirements

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Proven experience in providing social support, counseling, or case management, preferably with remote capabilities.
• Demonstrated proficiency in virtual communication tools (Zoom, Teams, etc.) and digital record‑keeping.
• Excellent active listening, empathy, and problem‑solving skills.
• Strong ability to build rapport and trust with clients remotely.
