Our client is seeking a highly organized and empathetic Remote Social Care Coordinator to join their fully remote team, serving the community in and around Durban. In this crucial role, you will manage client cases, coordinate support services, and ensure that individuals and families receive the assistance they need, all from a remote setting. You will be a key point of contact, providing guidance and support via digital platforms and phone, bridging the gap between clients and essential resources across KwaZulu-Natal.

Key Responsibilities

• Manage a caseload of clients, conducting virtual assessments and developing individualized care plans.
• Coordinate services with various healthcare providers, community organizations, and government agencies remotely.
• Provide telephone and video-based counseling and support to clients.
• Maintain meticulous digital records of client interactions, progress, and service delivery.
• Advocate for clients' needs and assist them in navigating complex support systems.
• Facilitate virtual support groups and online resource workshops.

Requirements

• Bachelor's degree in Social Work, Psychology, or a related field.
• Minimum of 3 years of experience in social work or case management, preferably with remote clients.
• Proficiency with telehealth platforms, video conferencing tools, and digital case management systems.
• Excellent virtual communication, active listening, and problem-solving skills.
• Strong understanding of social services and support networks relevant to the Durban area.
• Ability to work autonomously, manage time effectively, and maintain confidentiality in a remote environment.
