About the Role

Our client is looking for an experienced and organized Case Manager to join their expanding team, offering services remotely to clients in the Klerksdorp area. You will manage a caseload of individuals and families, providing essential support, advocacy, and connection to community resources. This role requires strong organizational skills, a deep understanding of social service networks, and the ability to build rapport and trust through virtual communication channels. As a Remote Case Manager, you will play a crucial role in ensuring clients receive timely and appropriate assistance, contributing to their stability and well-being from the convenience of your home office.

Key Responsibilities

  • Assess client needs, develop service plans, and coordinate care with various agencies.
  • Monitor client progress and provide ongoing support and advocacy.

Requirements

  • Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
  • Minimum of 4 years of experience in case management or social services.
  • Demonstrated knowledge of social service programs and community resources in the Klerksdorp region.
  • Proficiency in using case management software and standard office applications.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and manage multiple priorities in a remote setting.
