Telehealth GP / Nurse Practitioner

Weight Management | Health Optimisation | Men’s & Women’s Health | Mental Health

Horizon Health Care Partners Australia (HHCPA)

Australia-wide | 100% Remote | Contractor | Flexible Hours

Build a specialised telehealth practice around your clinical interests

Horizon Health Care Partners Australia (HHCPA) is an expanding Australian telehealth clinic providing accessible, patient-centred healthcare to patients across Australia.

As we expand our specialised clinical services, we are particularly interested in hearing from General Practitioners and Nurse Practitioners with experience, training or a strong clinical interest in Weight & Metabolic Health, Health Optimisation, Men’s and Women’s Health, Hormone Health and Mental Health.

We are looking for practitioners who want more than a high-volume general telehealth role. This is an opportunity to develop an ongoing patient caseload in areas of medicine that benefit from assessment, treatment planning, monitoring and continuity of care.

You can begin with a small number of sessions around your existing commitments and increase your availability as your HHCPA patient base develops.

Clinical areas we are particularly interested in

Depending on your qualifications, experience, scope of practice and clinical interests, your work with HHCPA may include:

Weight & Metabolic Health – assessment and medically supervised management of overweight and obesity, metabolic risk, lifestyle interventions, pharmacotherapy where clinically appropriate, pathology monitoring and ongoing treatment reviews.

Health Optimisation & Longevity – preventative health, metabolic optimisation, healthy ageing, recovery and evidence-informed health optimisation strategies.

Peptide Therapies – practitioners with appropriate knowledge, training or experience in peptide therapies and associated assessment, prescribing and monitoring requirements are particularly encouraged to apply.

Men’s Health & Hormone Health – men's health assessment, sexual health, testosterone/hormone-related presentations, appropriate investigations, treatment planning and ongoing monitoring within the practitioner’s scope of practice.

Women’s Health & Menopause – menopause and perimenopause care, reproductive and sexual health, hormonal health, appropriate investigations and ongoing management.

Mental Health – assessment, treatment and ongoing management of appropriate mental health presentations within the practitioner’s competence and scope of practice.

Practitioners may also elect to provide General Practice, chronic disease management, preventative healthcare, pathology and imaging referrals, medication management and other HHCPA telehealth services according to their individual interests and scope.

Why join HHCPA?

HHCPA provides the infrastructure and administrative support so practitioners can concentrate on clinical care.

We offer:

•

60–70% of practitioner billings, depending on practitioner type and agreed arrangement

Flexible days and session times

•

Ability to work around your existing practice, hospital or other commitments

•

Established telehealth systems and clinical workflows

•

Ongoing marketing and business development

•

Opportunity to develop a patient base within your areas of clinical interest

•

Ability to increase sessions as patient demand grows

•

Opportunity to contribute to new and developing HHCPA clinical services

•

Supportive management and an expanding Australia-wide healthcare model

About the role

You will provide high-quality telehealth consultations appropriate to your qualifications, experience and scope of practice.

Responsibilities may include conducting comprehensive patient assessments; reviewing medical history, current medications and relevant investigations; requesting appropriate pathology or imaging; developing individualised management plans; prescribing where clinically appropriate; providing follow-up care and treatment monitoring; educating patients about treatment options, benefits, risks and alternatives; maintaining accurate clinical documentation; and practising in accordance with applicable Australian professional, prescribing and telehealth requirements.

Clinical decisions remain the responsibility of the treating practitioner. No particular medication or treatment is guaranteed or expected to be prescribed.

Who we’re looking for

We would particularly like to hear from practitioners who have experience or additional training in one or more of the following areas:

Weight management and obesity medicine

  • metabolic health
  • GLP-1–based weight-management care

  • menopause
  • men's health
  • hormone health
  • testosterone-related care
  • mental health
  • health optimisation/longevity
  • peptide therapies
  • chronic disease management
  • preventative healthcare.

Applicants should hold appropriate qualifications and current registration relevant to their profession and role, have appropriate professional indemnity insurance, be confident providing telehealth consultations, demonstrate strong clinical decision-making and documentation, and practise within their individual competence and scope.

Previous telehealth experience is highly regarded.

Keep your current role and build telehealth around it

You do not need to leave your current practice or employment to join HHCPA.

Start with a few hours each week, nominate availability that works around your existing commitments and progressively increase your sessions as patient demand develops.

For practitioners with specialist interests in HHCPA's expanding services, there may be opportunities to take a larger role in developing these clinical streams as the clinic grows.

Remuneration

Contractor arrangement | 60–70% of practitioner billings

Earning capacity depends on practitioner type, consultation mix, appointment volume, consultation duration and availability.
